To book a tattoo appointment – please provide a well detailed description of your tattoo idea, including where it will go on your body and a rough size estimate for booking purposes. Someone from the shop will be in contact with you via email to further discuss. We recommend having reference photos ready that help to illustrate or support your idea – you will be asked for them when we contact you.

*For example*: I am looking to get a portrait of my dog on the upper right of my back – roughly 6″ x 6″. I have multiple photos I can send as reference……

We do not do in-shop consultations, so this form and subsequent emails will serve as your consultation. Be sure to include as much information as possible.

Jason did a very awesome job on my jack white piece and he is a very nice guy too he makes sure his customers are comfortable and he takes the time to put in a good product for you the only thing is that he is literally sooo awesome that he books up fast and you have to wait two months for an appointment but its totally worth it. Thank you creative ink and jason for what you do. I'm sure it isnt easy to become a great tattoo artist. Definitely the best shop I've been to!  Thank you
